51ÁÔÆæ End of Life Planning and Support Service (HELPSS) is for people living with a palliative diagnosis or a life-limiting condition, who may be approaching the last year of life. It aims to support patients to get the type of care that they want, at the time it is needed and in the place that is preferred.

Services

Advance Care Planning

Delivered by Saint Michael’s Hospice, the service supports patients and ensures their care choices at the end of life are discussed, recorded and shared with relevant healthcare professionals.

This includes:

  • What matters most to the patient, now and in the future
  • Their preferences for care and treatment
  • Where they would prefer to receive care
  • Choosing someone to speak on their behalf if needed
  • Recording of any legal decisions (such as refusing specific treatments)

Patients can also be referred to the Goldline through this service.

Goldline

Goldline is a 24/7 telephone service provided by Airedale NHS Foundation Trust.

It offers:

  • Clinical advice
  • Support and reassurance
  • Help for patient and their carers

Please note: Access to Goldline requires referral by a healthcare professional.

If you are a patient or carer:

  • Ask your GP, nurse or healthcare professional to refer you to Advance Care Planning or Goldline.

, through its partnership with , has provided up-front funding for HELPSS.Ìý Social Finance are experts in social investment and have been working with Macmillan, through the Macmillan End of Life Care Fund, to find new and innovative ways to improve services supporting people at the end of their lives.
